Advice and Guidance Survey

Introduction:

NHS Lincolnshire Integrated Care Board are looking for feedback from patients on their experience of Advice and Guidance.

Advice and Guidance is when your GP or practice team may contact a hospital specialist for advice about your care, rather than referring straight away.

Feedback will be used to help identify what is working well and where improvements can be made across Lincolnshire.

Accessing the Survey

The survey is quick and completely anonymous and only takes a few minutes to complete.
