Carer Support Needs Assessment Tool (CSNAT) Across Lincolnshire Survey

Introduction:

The Lincolnshire Carer Support Service is working with the PCN Transformation Team to develop a better understanding of awareness and use of the Carer Support Needs Assessment Tool (CSNAT) across Lincolnshire health and care services.

To help them understand current practice across Lincolnshire, they have developed a short survey to identify:

• Which services are currently using CSNAT.
• How the tool is being used in practice.
• Where it has been considered but not adopted.
• Any alternative approaches being used to identify and support carers’ needs.
• Opportunities to share learning and good practice.
