The Dementia 100 Assessment Tool helps to understand how well services are meeting the needs of people affected by dementia across prevention, diagnosis, post-diagnostic support, and end-of-life care.
It will inform future planning and help identify strengths, gaps, and opportunities for improvement at both place and system levels.
It is hoped to bring together a small group of 4–5 volunteers who can help complete the assessment collaboratively countywide as they need your local insight to ensure the tool reflects the reality of services and experiences in your area.
