What is Social Prescribing?
Our Social Prescribing team works collaboratively with colleagues across the health, care and third sector by linking people to community groups, organisations and services.
Our Link Workers take a holistic approach and offer personalised care allowing people time and space to make informed decisions about how they wish to move forward.
By doing this we hope to improve the health and wellbeing of those referred, reduce loneliness and social isolation as well as raising self-esteem and confidence.
We appreciate some people need more support than others to achieve their goals so offer different levels of interventions from signposting, solution planning and supported participation.
How is Social Prescribing Delivered?
CVS Lincolnshire deliver Social Prescribing on behalf of the Lincolnshire Integrated Care Board and as part of the Mental Health Transformation Project. The Integrated Care Board is a statutory NHS organisation who are responsible for developing a plan to meet health needs of the population, managing the NHS budget, and arranging for the provision of health services in a geographical area.
CVS Lincolnshire currently deliver two Social Prescribing pathways, Mental Health and Primary Care Network which is determined by us based on the referral information.
The Mental Health Pathway is a county-wide offer and is not determined by which GP Practice the person is registered with. The referral information for this is further down the page.
Our Primary Care Network Pathway is for selected GP practices across the county who have contracted with CVS Lincolnshire. We currently work with the following PCN areas Apex, Imp, East Lindsey, Meridian, South Lincoln Healthcare and Spalding.

What to Expect From Social Prescribing
Once referred we can help gather information, set goals, signpost or refer into interests such as arts and craft groups, peer support and friendship groups, volunteering opportunities and outdoor activities. We often meet people in their community at a safe place where they can freely talk to their link worker.
Link workers will also be able to connect people to services that can provide practical help and advice relating to food insecurity, accessing technology, benefits and debt advice and mental health support
