Wellbeing Lincs Service

Introduction:

Wellbeing Lincs is a countywide service, funded by Lincolnshire County Council offering support to adults across Lincolnshire to help them to achieve confident, fulfilled and independent living.

It offers a helping hand through life’s changes, which might include:

  • Ill health
  • Disability
  • Changes to financial circumstances
  • Changing medical need
  • Bereavement
  • Lack of family support
  • Loneliness and social isolation
  • Change in general situation circumstance

As a preventative service, Wellbeing Lincs aims to:

  • Enhance wellbeing
  • Improve people’s access to support services
  • Reduce and delay escalation to statutory services
  • Provide direct support and signpost access to community resources

The service is provided completely free of charge.

Who Can Use The Service?

Anyone over the age of 18 can access the service providing you meet at least four of the following eligibility criteria:

  • Unable to manage long term health / medical conditions
  • Regularly visit the GP for the same medical condition or for non-medical reasons
  • Unplanned hospitalisation or A&E attendance in the last 90 days
  • Accessed social care services in the last 12 months including: assessment, day care, home care, re-ablement or residential care services
  • Bereavement (spouse / partner) or divorce in the past 12 months
  • Had a fall in the past three months, either at home or away from the home
  • Unable to manoeuvre around the home safely
  • Lacks social support and/or interaction with family, friends or carers, or feels isolated
  • Feel stressed, depressed or anxious
  • Unable to sustain work, education, training or volunteering
  • Unable to manage money or in considerable debt
  • Poor lifestyle management and behaviours which impact on overall health and wellbeing.
  • Aged over 65 years old

If you are supported by a charity, support agency or other national or local organisation they can contact us on your behalf.

Important Referral Information

The Lincolnshire Wellbeing Service is now accepting online requests for support. These requests will be administrated by Lincolnshire County Council Customer Service Centre.

To access the service a customer must:

  • Be 18 years old or over
  • Live in Lincolnshire
  • Be at risk of becoming unable to live independently at home, increasing a need for ongoing services.
  • Have barriers to accessing information and written advice to help themselves or have challenges that stop them from being able to interpret or act on the information and advice that would enable them to help themselves.

In addition, customers must need additional support with at least one of the needs-based outcomes to live independently at home and maintain good health and wellbeing.

Customers can now complete a request for support form online by following this link to the Lincolnshire County Council website, or by calling 01522 782140 to have a conversation about service eligibility.

Once accepted into Wellbeing Lincs customers will benefit from a strengths-based assessment, from one case officer, meeting all identified needs in a combined assessment and support plan, improving the customer experience overall.
